EMPOWER HOME LLC is preparing to introduce residential/group home services for adults age 18 and older who need assistance with daily living due to physical, mental-health, or age-related conditions. The planned program is intended for individuals who may need support with activities of daily living, health-related needs, behavioral support, community participation, or other areas that can be safely supported within the program’s approved level of care.
Planned Residential Services & Supports
The planned residential program is designed to provide day-to-day support across several areas of an individual’s routine and community participation:
24/7 Supervision and Residential Support
Ongoing supervision and residential assistance within the planned program setting.
Activities of Daily Living (ADLs)
Assistance with everyday activities based on an individual’s support needs.
Medication Support and Management
Medication-related support within the program’s authorized scope.
Health Monitoring
Attention to health needs and changes that may require appropriate follow-up.
Physical, Occupational, and Speech Therapy
Access and coordination for therapy services when needed.
Behavioral and Counseling Support
Support related to behavioral and counseling needs within the program’s scope.
Life-Skills Training
Assistance with developing and practicing skills used in everyday activities.
Educational and Vocational Support
Support related to education, work, and vocational activities.
Recreational Activities
Opportunities for appropriate recreation and participation in activities.
Community Integration
Support for participation in community activities and settings.
Care Coordination
Coordination of services and supports relevant to the individual’s needs.
Transportation
Transportation to medical appointments, work, school, therapy, and approved community activities.
Who the Planned Program Will Serve
The planned program will be intended for:
- Adults age 18 and older
- Individuals who need assistance with activities of daily living
- Adults with physical support needs
- Adults with mental-health support needs
- Adults with age-related support needs
- Individuals whose needs can be safely supported within the program’s approved level of care
The program is intended to provide support based on the individual’s circumstances while maintaining an appropriate level of supervision and assistance within its approved scope.
Planned Staffing Structure
The planned residential program is expected to include administrative, program, nursing, and direct-support roles:
- Chief Executive Officer (CEO)
- Program Director
- Accounts Coordinator
- Registered Nurse (RN)
- Certified Nursing Assistants (CNAs)
These roles are part of the planned staffing structure for the residential/group home program and reflect the different responsibilities anticipated within the service model.
